Application for Ballot by Mail

Eligibility

  • Any registered voter of the age of 65 or older
  • Any registered voter with a disability
  • Any voter who will have an expected absence from the county during Early Voting and Election Day (Ballot must be mailed to an address outside the county. Cannot be mailed to the voter’s residence inside the county.)
  • Confinement in Jail

Annual Application

Voters who are 65 or over or have a disability may choose an Annual Application (box 6A) You will automatically receive a ballot for each election that you are eligible.

Things to Know

  • If the application is submitted by fax or email, the applicant must also submit the original application by mail not later than the 4th business day after submitting the faxed or emailed application
  • The deadline that your application must be received in this office is always the 11th day before Election Day
  • You may fill out an application English, application Spanish (PDF), print it, sign it, and send it to us. You can send it in by emailing the Election Administrator, faxing it to 806-656-2151, mail it to one of the following addresses:

  • You must choose a party if you want to vote in a primary election
  • Your phone number and/or email is not required but is very useful if we have any questions and could help avoid a delay in processing your request
  • Your signature is very important. You must sign the application the same way that you will sign your ballot envelope for your vote to count
